This is being offered by Embroidery Library. You make as many as you wish and then forward them to the PTA.

This download is no longer available.
However, this site - http://totallystitchin.n...hook-elementary has downloads, but I do not know for how long. The deadline for submitted to the PTA is indicated as 12/21/13.
